Hospice Care

Hospice care is for patients whose illness is no longer responding to treatment. Every at-home hospice care plan is designed to honor the patient’s wishes and values, providing support for physical comfort, emotional well-being, and spiritual peace.

There are a lot of factors to consider when reacting to a new or difficult diagnosis. The Care Team is here to provide the support and answers you need to move forward.

Palliative care includes specialized care for patients still pursuing treatment for a serious or chronic illness and do not have a terminal diagnosis. Palliative care is focused on managing pain and symptoms and reducing stress. 

Hospice Care benefits any patient faced with a life-limiting illness. There is a focus on quality of life, emotional, spiritual, and physical needs, and making the last days to months of life as meaningful as possible for the patient and their loved ones.

In-home hospice and palliative care are covered by Medicare and most commercial insurance plans, including BCBS, Humana, Aetna, United, Priority Health, and others. If you’re unsure what your coverage includes, give the Care Team a call. We’ll help you navigate your options before you make any decisions. 

The Care Team provides palliative and hospice care at home, whether that is a private residence, long-term or assisted living facility, or independent living facility. Outpatient hospice care is provided in select service areas across Illinois, Indiana, Michigan, Ohio, and Pennsylvania. Outpatient palliative care is provided in select service areas across Illinois and Ohio.
