What is ASAP?

ASAP (Anticipatory Symptom Assessment Pathways) is a proactive hospice support program that helps families recognize and respond to common symptom changes related to serious illness.

Through guided education and simple, easy-to-follow tools, we equip caregivers with practical steps they can take while staying closely connected to our clinical team. You are never expected to manage symptoms alone. We simply help you feel more prepared.

A Simple, Clear Symptom Guide

Families enrolled in ASAP receive a straightforward symptom management guide developed by hospice physicians and clinical experts.

The guide helps caregivers recognize and respond to changes such as:

  • Changes in breathing
  • Pain
  • Nausea or vomiting
  • Constipation
  • Fever
  • Increased confusion
  • Anxiety or restlessness
  • Changes in appetite
  • Rapid decline or noticeable changes in condition
  • And many other common end-of-life symptoms.

It provides step-by-step guidance along with clear direction on when to call us.

Under physician direction, our interdisciplinary team — nurses, aides, social workers, chaplains, and volunteers — remains actively involved in every stage of care. If symptoms escalate or new concerns arise, our team is available 24 hours a day, 7 days a week. We respond quickly so families can stay focused on comfort and presence.
