Respite Support

Caregiving is a demanding job, physically and emotionally. Respite care gives you scheduled time to rest and recharge.

Respite care involves short-term facility-based care. You will also have a large support network of social workers, therapists, volunteers, and trained medical professionals to support the patient at home.

Family support services are provided alongside any palliative or hospice care at no additional cost. Insurance coverage may vary depending on your plan, but hospice and palliative services are generally covered by Medicare and most insurance plans. Our care coordinators can help answer any questions.

Hospice bereavement services are about helping you during one of the most challenging times in your life. No matter your exposure to loss or familiarity with the grieving process, it’s never easy. Some days you’ll feel the full range of emotions — from sadness, denial, and anxiety to isolation, anger, and even guilt. And we’ll be right by your side.

We provide these and other hospice bereavement services:

  • A listening ear
  • Assistance with logistics needed at the end of life
  • Counseling
  • Education about the grieving process
  • Gentle guidance and advice
  • Help finding ways to cope and honor your loved one’s memory
  • Referrals for other community resources

Anticipatory grief refers to all of the feelings someone may experience before a loss occurs. Grief manifests in many ways, and anticipatory grief may involve mourning ongoing changes, active declines, future absences, and more.

Grief is not a linear process—whatever you are feeling is completely normal, from sadness to anger. The Care Team offers families access to chaplains, therapists, and bereavement counselors who are all trained to help you navigate the death of your loved one.

The Care Team provides bereavement support to families for up to 13 months following a patient’s death. Our counselors will check in with you, offer ongoing support, and connect you to additional grief resources as needed.

Palliative Care:

For anyone with a serious illness at any stage and may be provided alongside treatments intended to cure or slow disease progression.

  • Can be provided at any stage of serious illness
  • Can be given alongside curative or life-prolonging treatments
  • Focuses on symptom management
  • Helps patients understand treatment options and make informed decisions
  • Supports both patients and families emotionally and spiritually

 

Hospice Care:

For those choosing to focus on comfort, symptom relief, and quality of life rather than continuing curative treatment.

  • Begins when the treatment plan shifts to symptom management, pain control, and comfort. Cure is not medically possible.
  • Provides expert management of pain and symptoms
  • Offers emotional and spiritual support for patients and families
  • Includes bereavement support for loved ones after death
  • Helps patients live as fully and comfortably as possible

When your loved one receives hospice care from The Care Team, they will receive compassionate care focused on improving their quality of life and centered around their hobbies and interests. From helping patients stay involved with their favorite activities, to facilitating end-of-life preparations and management of day-to-day care, our team is with you.

In addition to hands-on care, The Care Team provides all medications, medical equipment, and medical supplies that are related to the patient’s primary disease.
