In-Home Palliative Care

Helping Patients & Their Families Live Life Fully

Palliative care provides comfort, relief, and support for people living with serious or chronic illness.

Palliative care can be provided alongside curative treatment, helping patients and their families manage symptoms, reduce stress, and focus on what matters most.

We know that the decisions and conversations surrounding hospice can be confusing and difficult. Choosing hospice doesn’t mean giving up, it means getting the support needed to improve comfort and quality of life.

Palliative Care vs. Hospice Care

Both palliative and hospice care are focused on comfort, support, and quality of life.

Palliative care is for anyone with a serious illness at any stage, and may be provided alongside treatments intended to cure or slow disease progression.

Hospice care is for those choosing to focus on comfort, symptom relief, and quality of life rather than continuing curative treatment.

We’re Here to Answer Your Questions

These are the questions we hear most often. If yours isn’t here, give our team a call.

Palliative care is for anyone living with a serious or chronic illness, at any stage of disease. If you or a loved one is managing symptoms from conditions like cancer, heart disease, COPD, kidney disease, Parkinson’s, or another serious illness, palliative care may be able to help. The best way to find out if it’s the right fit is to have a conversation with our team about your goals and what you’re experiencing right now.

Palliative care is designed to work alongside your existing medical treatment, not replace it. You can continue seeing your specialists and pursuing treatment for your illness while our team focuses on managing your symptoms, reducing discomfort, and supporting your overall quality of life. Our nurse practitioners coordinate directly with your doctors to make sure your care is connected and consistent, so nothing falls through the cracks.

Palliative care comes to you. The Care Team provides in-home palliative care services wherever you call home — whether that’s a private residence, a family member’s home, an assisted living community, or a skilled nursing facility. You shouldn’t have to travel to get the support you need, and we’re committed to meeting you where you are.

At-home palliative care is covered by Medicare and most commercial insurance plans, including BCBS, Humana, Aetna, United, Priority Health, and others. If you’re unsure what your coverage includes, give the Care Team a call. We’ll help you navigate your options before you make any decisions.

If you or someone you love is living with a serious illness and struggling with pain, breathlessness, fatigue, anxiety, or the weight of managing complex care, palliative care may help. The earlier you understand your options, the more empowered you’ll feel. If you’re not sure, call us — we’ll have an honest conversation about what makes sense for your situation.

Many palliative care services are billed through standard Medicare Part B benefits, which can include nurse practitioner visits, symptom management, and care coordination. Our team is experienced in working within Medicare guidelines and can help clarify what’s covered for your specific situation.
