Is It Time to Consider Hospice?
Hospice care typically begins when a physician determines a life expectancy of six months or less assuming disease runs its normal course, and the patient has decided to stop curative treatment. If your loved one improves, they can always leave hospice.
We know that the decisions and conversations surrounding hospice can be confusing and difficult. Choosing hospice doesn’t mean giving up, it means getting the support needed to improve comfort and quality of life.

Who is eligible to receive hospice care at home?
Hospice eligibility is based on a medical prognosis, or life expectancy, of 6 months or less if the illness follows its expected course. Patients with a wide range of diagnoses may qualify, including cancer, heart disease, lung disease, Alzheimer’s, and other serious conditions.
At the Care Team, we make it a priority to have a conversation about goals of care: where is your loved one in their illness, and what matters most to them now? If you are unsure whether your loved one may qualify, reach out to us.
Does hospice care provide my medication, equipment, and supplies?
Yes, everything your loved one needs for comfort and care is coordinated through our team and delivered to wherever they call home.
As part of your hospice benefit, The Care Team provides the medications, medical equipment, and supplies needed for care. That means you will not be navigating pharmacy orders or equipment rentals on your own.
What if I have a problem at night or on weekends?
The Care Team has an expert team of nurses who are available 24 hours a day, seven days a week, including nights, weekends, and holidays. You will never be left waiting until Monday morning.
Can my loved one leave hospice if they improve?
Yes. If your loved one’s condition improves or stabilizes, they have the right to be discharged from hospice and transition back to curative treatment. Choosing hospice is not a one-way door, it is a decision based on current goals and current health, and those goals can evolve. Our team will always support what is best for your loved one, wherever that path leads.
Will insurance cover hospice care?
Hospice care is 100% covered by Medicare and Medicaid, and is typically covered by most commercial insurance plans with potential copays.
Where is hospice care provided?
The Care Team provides hospice services at home. Our team comes to you, wherever it is that you call home. This includes private residences, independent living facilities, or assisted living facilities.
How quickly can care begin?
We respond to care inquiries within hours and can offer same-day admissions. Our goal is to make sure every patient has the support they need as soon as possible.
What hospice support for families is available?
In-home hospice care from The Care Team is designed to support the whole family, not just the patient. Our interdisciplinary team includes registered nurses, home health aides, social workers, chaplains, therapists, and trained volunteers — each playing a role in caring for the patient and supporting the people who love them. We also provide bereavement counseling for family members after a loved one passes. From the first call to the weeks and months that follow, we are here for your entire family.
How is hospice care different from palliative care?
Palliative Care:
For anyone with a serious illness at any stage and may be provided alongside treatments intended to cure or slow disease progression.
