Leads your interdisciplinary team to develop the holistic care plan.
Attending Physicians
Certifies the patient’s diagnosis, assesses the patient’s needs, and directs the plan of care together with the hospice team.

What is the Difference Between Palliative & Hospice Care?
Both palliative and hospice care are focused on comfort, support, and quality of life.
Palliative care is for anyone with a serious illness at any stage and may be provided alongside treatments intended to cure or slow disease progression.
Hospice care is for those no longer responding to curative treatment and is focused on comfort, symptom relief, and quality of life.

Are your chaplains tied to a specific religious belief or affiliation?
No. Our chaplains provide spiritual and emotional support tailored to the patient and family’s own beliefs, values, and needs, including no religious belief at all. Participation is entirely optional and is always on your terms.
What exactly do volunteers do, and are they trained?
Our volunteers are dedicated to creating meaningful moments, offering comfort, and providing empathetic support to patients and their families. Volunteers do not provide patient care. They offer the gift of companionship through activities, sharing stories, or just sitting together.
All volunteers are required to complete a background check. We also offer training and ongoing support to ensure that our volunteers feel confident and equipped to create a caring and compassionate environment for our patients and their loved ones.
Will the palliative or hospice care team communicate with my existing medical provider?
Absolutely. The Care Team is a trusted hospice partner for physicians, hospitals, and residential facilities. Our staff works closely with any referral sources or existing medical providers to ensure care is always tailored to the patient and their unique needs. Our experienced hospice professionals provide comfort-focused medical care, symptom management, and emotional support for patients facing a life-limiting illness.
